Here’s what you can expect when you choose in-home hospice care:

Visit & Assessment

A hospice team member will visit your home, assess your needs, advise you on any medical equipment needed and help you make arrangements to obtain it.

Care Plan Creation

The Hospice care team will create an individualized care plan for you. This helps everyone involved understand the amount of caregiving you require and determine the best way to provide it for you.

Assigned Nurse & Team

The care plan will designate the team RN who visits your home. Depending upon your needs, your team will include a nurse and possibly a volunteer when requested.

Regular Visits & Support

Hospice team members will visit you regularly per your care plan and are always accessible by phone if questions arise.
